Digital Therapeutics for Vergence

An emerging class of technology-assisted interventions for binocular vision disorders, particularly relevant to acute-acquired-concomitant-esotropia and related vergence dysfunctions. 1

Approaches

  • VR-based binocular therapy — Immersive environments with controlled disparity to retrain vergence
  • Computerized vergence rehabilitation — Adaptive difficulty, quantitative progress tracking
  • Adaptive disparity training — Algorithm-adjusted disparity to operate at the patient’s fusion threshold
  • Eye-tracking-guided fusion exercises — Real-time gaze-contingent feedback

Therapeutic Goals

  1. Restore divergence fusional reserves
  2. Rebalance convergence/divergence control
  3. Promote cortical binocular plasticity
  4. Provide quantitative outcome metrics

Status

This field is early-stage but highly promising. Unlike traditional vision-therapy (which relies on subjective patient report and therapist observation), digital therapeutics offer:

  • Objective eye-tracking data
  • Standardized protocols
  • Home-based delivery
  • Continuous monitoring

Limitations

  • Limited clinical trial data
  • Equipment cost and access
  • Unknown long-term durability
  • No established regulatory framework
